29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employee who performed servicing and maintenance on or around the Somerset Dough Stretcher Model # CDR-500 machine was not required to (1) Physically locate and operate the electrical disconnect in such a manner as to isolate the machine or equipment from the energy source in that on January 10, 2016 the dough stretcher was no turned off and unplugged. (2) Affix lockout or tagout devices to the electrical disconnect.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) The employer did not report the hospitalization of an employee on January 10, 2016 that was injured and then was admitted into the hospital.
