29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On January 19, 2016 employees engaged in steel erection activities were exposed to caught-in and struck-by hazards while operating a telehandler. Hardware was utilized to hold the forks to the mast that did not meet the manufacturer's specifications.
