Site: 146 Wilson Street Brooklyn, NY  On or about 12/14/15  a)  Employees worked in an excavation approximately 13 feet deep which did not have underpinning or other means installed to ensure stability of an adjacent walls or structures.  Employees were hand digging an excavation and there was no shoring, bracing, or underpinning provided to support and stabilize the foundation of the adjoining property.

Site: 146 Wilson Street Brooklyn, NY  On or about 12/14/15  a)  Employer did not provide daily inspections by a competent person of an excavation which employees were working.  The excavation in a back yard was being hand dug and was approximately 13 feet deep.  Employees were trapped when a cave-in occurred.
