29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT) On or about January 13, 2016 while attempting to discover the source of a noise inside the feeder table and roller, an employees removed the guard covering roller and failed to let the roller come to a complete stop before reaching into the point of operation. The employee was exposed to a caught by hazard when his gloved hand was caught on the fine teeth of the roller and suffered an amputation of his right index finger.

29 CFR 1910.212(a): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about January 13, 2016 while attempting to discover what the noise was inside the machine, the employees working at the machine turned to machine off and lifted the guard that covered the rollers within. Not waiting for the roller to stop an employee reached into the rotating in going nip point and suffered an amputation of his right index finger the second knuckle.
