Warehouse located at 8423 Sunstate St. Tampa (FL) were exposed to falls while using a Werner 16 foot fiberglass ladder which was fully extended, placed on top of a wooden box with a slide guard in front and a piece of wood behind its feet so that the ladder would not slip, on or about 01/20/2016.
