29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) 7 Bryant Park, northeast corner of building, on or about 1-12-16:  Two employees were accessing and egressed a scissor lift by climbing the rails of lift, exposing employees to a fall hazard of 24 feet 1 inch.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) 7 Bryant Park, northeast corner of building, on or about 1-12-16:  Two employees were working on a canopy type roof, without fall protection, exposing the employees to a fall hazard of  21 feet 7 inches.
