(h) KMX CNC Lathe Machine T-23: Employees operated a KMX CNC Lathe that did not have the rotating cutting tools guarded. The sliding door that enclosed the milling process and other rotating parts had the interlock bypassed exposing the operator to a caught-in/on hazard.    (i) Miyano CNC Lathe Machine T-26: Employees operated a Miyano CNC Lathe that did not have the rotating cutting tools guarded. The sliding door that enclosed the milling process and other rotating parts had the interlock bypassed and glass on the sliding door removed exposing the operator to a caught-in/on hazard.

Training was not provided regarding the Hazard Communication 2012 standard and new Global Harmonization (GHS) label elements (product identifier, signal word, pictogram, hazard statement(s), and precautionary requirements) and new safety data sheet (SDS) format.
