29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: In the maintenance shop area - On or about January 13, 2016, the tongue guards on the pedestal mounted bench grinders were not adjusted to less than 1/4 inches from the abrasive wheels; a) The WV20492 grinder tongue guards did not cover the full width of the opening from left to right on each side of the tongue guard and the right tongue was missing. b) The WV00071 grinder tongue guards did not cover the full width of the opening from left to right on each side of the tongue guards.
