The orange and black extension cord used to supply power to computer equipment.   2. The orange extension cord that was spliced into the power cord of the Prime 12 outlet strip. (Item Corrected)

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a) Facility, where electrical equipment not used as listed or labeled included:  1. Administration Cubicle, where a relocatable power tap was used to supply power appliances including the Lasko floor heater.   2. Third Floor Studies Area, where a relocatable power tap was daisy chained with an extension cord and used to supply power to the Vano light table.  The U.S. Geological Survey was previously cited for a violation of this standard which was contained in OSHA inspection no. 1079489, citation no. 1, item no. 2, issued on September 4, 2015 with respect to a workplace at Ferndale, WA.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:  a) Facility, where exit accesses were not maintained:  1. Second Floor IT Storage Room, where the placement of boxes reduced the aisle way width to 21 inches.   2. Second Floor IT Storage Room, where the placement of metal file cabinets reduced the aisle way width to 25 inches.  3. Second Floor Cubicle near the kitchen, where the placement of the storage cabinet reduced the aisle way width to 22 inches.  4. Note Sheet Back Files and Data Section, where the placement of the file cabinets reduced the aisle way width to 23 1/2-26 inches.
