On or about January 21, 2016 employees were cutting old roof decking and cutting new plywood to size, and using pneumatic nail guns to secure the new decking, and were not protected from eye or face injuries from flying debris because they were not wearing proper safety glasses or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a). On or about January 21, 2016 employees were stripping an existing residential roof of old shingles, underlayment and sheathing and were not protected from falling 12 feet to the ground below, because they were not protected by any means of safety nets, guard rails, or complete personal fall protection harness, ropes, and roof anchors.
