29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. The employer's lockout/tagout program did not include documented machine-specific procedures for servicing and maintaining production machines and equipment including the Accurshears and Chicago Press located in the establishment. This exposed employees to caught-in hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. The employer failed to guard the point(s) of operation of machinery to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s) on the following machines: a. Accurshear S/N 2472- The guarding on the shear was not adequate to prevent employees from gaining access to the point of danger when the shear was in operation. This exposed employees to a crushing and shearing hazard. b. Chicago Press S/N P-9348- The point of operation of the press was not guarded to prevent employees from gaining access during operation. This exposed employees to a crushing hazard. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer did not report an amputation as a result of a work-related incident to OSHA within twenty-four (24) hours. Abatement is not required for this item.
