29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. At the jobsite, fall protection was not provided to employees doing framing work and installing prefabricated exterior walls on the 2nd floor, exposing the employees to an 11 foot 11 inch fall hazard, on or about 1/21/2016.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. At the jobsite, the portable extension ladder used to access the second floor was not secured from displacement, exposing employees to an 11 foot 11 inch fall hazard, on or about 1/21/2016.
