29 CFR 1910.106(e)(6)(i): "General." Adequate precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:    On or about January 16, 2016, in the facility, a maintenance employee required to clean/strip/wax a production office floor was exposed to a burn hazard when using a highly flammable liquid (Typewash 50/50) with an unapproved (unclassified) electric motor floor scrubber.  Adequate precautions were not taken to prevent the ignition of flammable vapors.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    On or about January 16, 2016, in the facility, a maintenance employee required to clean/strip/wax a production office floor was exposed to a burn hazard when using a highly flammable liquid (Typewash 50/50) with an unapproved (unclassified) electric motor floor scrubber.  The employee had not been provided with effective information and training on the hazards of the typewash including, but not limited to, flammability severity and potential ignition sources.    Royal Envelope Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 405822, citation number 1, item number 2 and was affirmed as a final order on August 21, 2012, with respect to a workplace located at 4114 South Peoria Street, Chicago, IL. 60609.

29 CFR 1910.134(c)(2)(ii):  The employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks):  On or about January 16, 2016, in the facility, a maintenance employee required to clean/strip/wax a production office floor wore a half-mask, air-purifying respirator voluntarily.  The employer did not establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that it's use did not present a health hazard to the user.
