29 CFR 1910.23(d)(1)(iii): Flight(s) of stairs with 4 or more risers, less than 44 inches wide and having both sides open were not equipped with one standard stair railing on each side:  On or about1/21/16 in the production department on the east side; the flight of stairs (five risers, 27" wide) leading to the Extruder machine did not have hand rails on either side; thus exposing employees to a 6' fall from the top riser.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation:  On or about 1/21/16 in the Warehouse building; the employer allowed the other employees to be exposed to the unguarded blade of the hydraulic shear (no model and serial numbers) where the opening is 8' wide on the north and south sides and 12' tall, and the guillotine blade is 6.5' tall; thus exposing employees to fatal injuries.
