29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  a) Cooper University Hospital One Cooper Plaza, Camden, NJ 08103: The employer failed to ensure that an employee working in the Emergency Department and required to wear a 3M 1860 N95 respirator in the event of treating a patient with suspected tuberculosis received a fit test prior to using the respirator, on or about 1/11/16.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
