On 01/19/16 an employee's finger was amputated. b) Loading dock: The employer failed to report an accident resulting in in-patient hospitalization within 24 hours. On 02/16/16 an employee had to be taken to a hospital when he bruised his leg while operating electric pallet lift. c) Meat department: The employer failed to report an accident resulting in in-patient hospitalization within 24 hours. On 03/30/16 An employee had to be taken to a hospital when he cut his finger while operating a band saw.
