29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about January 12, 2016:  a) Employees installing roof sheathing with pneumatic nail guns and hammers were exposed to eye injuries when the employer failed to ensure that they were wearing safety glasses while performing this task.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On or about January 12, 2016  a) Employees were exposed to electrical shock and/or burns while utilizing a four way electrical outlet box to power various equipment, the four way electrical box is not designed to be used in this manner.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 12, 2016:  a) Employees installing roof sheathing were exposed to fall hazards of approximately 19.4 feet when the employer failed to ensure that employees were using fall protection measures to reduce or eliminate the fall hazard.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
