29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: On or about January 22, 2016, employees were required to perform minor servicing and maintenance on the Flow Master Machines by clearing jams and adding water to tanks on the machines, and there were no machine specific procedures developed and utilized for the control of hazardous energy to protect employees. Thereby exposing employees to hazards associated with the unexpected start up of machinery.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about October 22, 2015, employees were operating the Lable Aire Table and the motor on the machine was not guarded to protect employees from ingoing nip points on the machine.
