29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a). Worksite located at or near 7128 N. 50th Street, Kansas City, Missouri- Employee exposed to a fall hazard was observed working out of an aerial lift and did not have his leg straps connected to his body harness, thus system cannot operate the way the system was designed.
