29 CFR 1910.36(e)(1): A side-hinged door was not used to connect any room to an exit route:      (a)    At the heavy equipment pole barn, on or about 1/22/2016:  Employees worked inside and inspected or maintained heavy equipment in the barn/garage, where there were two large sliding garage doors but no side hinged personnel door, for use as an emergency exit and this exposed employees to entrapment in an emergency.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:      (a)    At the laboratory, on or about 1/22/2016:  The laboratory employees handled and used acids, bases, aldehydes and the safety shower had not been tested in years to insure adequate water flow, therefore in an emergency the laboratory workers were at risk of chemical burns.  Abatement certification must be submitted.
