29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) On or about January 22, 2016, employees were using a portable Hitachi Brand hand held electric grinders, and a 10" DeWalt hand fed rip saw, and were not protected from potential eye or face injuries from physical impact because they were not wearing any eye or face protection.

29 CFR 1926.304(i)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw blade above the material being cut. a) On or about January 22, 2016 employees were using a DeWalt brand portable 10" hand fed rip saw and were exposed to amputation injuries because the saw blade was not guarded by an adjustable blade guard or hood as per manufacturers design.
