29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   On or about 12/16/15 located in the Process Building, CREEL area Room Vestibule -2:  Two employees were climbing the rails / edge of the areal lift to get up in to the ceiling bar joist to install the plumbing and chill water lines onto the building at approximately 20- feet above the lower level and exposed to a fall hazard of up to 20.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about 12/16/15 located in the curing building pit area NW corner: An employee was exposed to fall hazards, while performing pipe work, the employee was using a job site made portable pipe extension ladder to gain access to the 1st floor, and the ladder was 8- inches below the landing surface.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    On or about 12/16/15 located in the curing building pit area NW corner: An employee was exposed to fall hazards, while performing pipe work, the employee was using a job site made portable pipe extension ladder to gain access to the 1st floor, with structural defect bending in the middle.
