29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) 753 East Warrington Avenue, Pittsburgh, PA 15210: On or about January 15, 2016, the employer did not report an in-patient hospitalization to OSHA within twenty-four (24) hours. An employee was hospitalized on January 15, 2016 after being injured while operating a Milwaukee Sawzall brand reciprocating saw. The employer did not notify OSHA of the in-patient hospitalization until the afternoon of January 20, 2016.
