29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:  a) Cooper University Hospital Emergency Department: Favorite Healthcare Staffing did not ensure that employees were fit tested on the same make and model of N95 respirators used in the facility, including 3M 1860 and Moldex 1513, on or about 1/8/16.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
