OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazards:    (a) Melt Shop, Number Five Electric Arc Furnace: On or before January 20, 2016, employees were exposed to explosion hazards in that inspections of all internal cooling panels between heats were not completed.     (b) Melt Shop, Number Five Electric Arc Furnace: On or before January 20, 2016, employees were exposed to explosion hazards in that cycles for the main water safety shut off valve in the hydraulic room were not accounted for.    (c) Melt Shop, Number Five Electric Arc Furnace, Panel 12, Bottom Circuit, Feed Side: On or before January 20, 2016, employees were exposed to explosion hazards in that the quarter turn ball valve was not turned on in accordance with the manufactures instructions.     (d) Melt Shop, Number Five Electric Arc Furnace: On or before April 11-12, 2016, employees were exposed to explosion hazards in that inspections of all internal cooling panels between heats were not completed.     (e) Melt Shop, Number Five Electric Arc Furnace: On or before April 11-12, 2016, employees were exposed to explosion hazards in that cycles for the main water safety shut off valve in the hydraulic room were not accounted for.    (f) Melt Shop, Number Five Electric Arc Furnace: On or before April 11-12, 2016, employees were exposed to explosion hazards in that the cooling panels quarter turn ball valves were wire tied in the open position. The manufactures instruction requires that the valve handle be removed after water flow is proven.
