29 CFR 1926.251(b)(6): In addition to the inspection required by other paragraphs of this section, a thorough periodic inspection of alloy steel chain slings in use were not made on a regular basis, to be determined on the basis of frequency of use;severity of service conditions; nature of lifts being made; and experience gained on the service life of slings used in similar circumstances. Such inspections were in no event made at intervals greater than once every 12 months:  a)  At Wetumpka Airport, Wetumpka, Alabama:  On or about January 20, 2016 and at times prior:  the employer failed to ensure alloy slings used on site were properly inspected to determine things such as but not limited to severity of service condition, frequency of sling use and to ensure slings had a rating identification tag on them exposing his employees to struck-by and crushing hazards.
