29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  Employees required to weld in the welding area were not informed of the hazards of the metals, when heated, and the chemicals used while completing welding job orders. Employees were exposed to chemicals such as but no limited to copper coated Metal Inert Gas (MIG) Wire (inhalation hazard-respiratory system when heated- metal fume fever, body aches, nausea), Carbon Steel Welding Wire(inhalation hazard-respiratory system when heated- metal fume fever, body aches, nausea), Methyl Ethyl Ketone (MEK)(flammable hazard-burns,  absorption hazard - skin itching, redness and irritation, headache, dizziness ).

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On January 22, 2016, the employer did not provide a written certification certifying the workplace evaluated; the person certifying that an evaluation was performed; the date(s) of the hazard assessment; nor the document identified as a certification of hazard assessment. Employees are exposed to respiratory, eye irritation, and skin contact hazards, and are required to wear safety glasses, safety shoes, welding helmets, leather gloves, and flame retardant clothing.
