29 CFR 1926.451(h)(1): In addition to wearing hardhats, each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects: On or about January 21, 2016, employee(s) were exposed to the hazard of being struck by falling objects, while working from a scaffold which did not have additional protection to prevent material from falling to the ground.

29 CFR 1926.452(c)(2): Frames and panels were not braced securely by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. All brace connections were not secure: On January 21, 2106, employees were exposed to the hazard of a fall of over 10 feet to the ground level below, while laying stones on the exterior wall of the structure from the second tier of a tubular welded scaffold, which was attached to the frame with wire.
