29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On or about January 27, 2016, employees at this location were engaged in hydrogen production and purification and had potential exposure to carbon monoxide due to packing leaks on process valves found within the PSA skid. The employer did not perform air monitoring of employees to assess employee exposure to carbon monoxide gas.

The cited standard is amended to: 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not en previously trained about was introduced into their work area: The AVD is amended as follows: On or about January 27, 2016, employees at this location were engaged in hydrogen production and purification where facts establishing an other-than-serious violation of 29 CFR 1910.1200(h)(1) existed with respect to process valves found with the PSA skid.
