29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): (a) West side of Mechanical Room (B-17): Extension cords (carrying 120 volts) were used in lieu of fixed wiring to provide power to built in multiple outlets installed in two adjacent non-portable work benches and to an overhead florescent light fixture mounted to the ceiling above the work benches. (b) East side of Mechanical Room (B-17): A section of orange extension cord (carrying 120 volts) was used in lieu of fixed wiring to provide power to a hanging florescent light fixture mounted above a work bench. The extension cord was wired between an overhead junction box and the light. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1001(j)(5)(i): The building owner or employer did not affix or post labels or signs to notify employees of what materials contain ACM and/or PACM for previously installed ACM and/or PACM: (a) The employer did not affix labels on asbestos containing thermal system insulation present on overhead hot water supply and return lines located in the mechanical room (Room B-17) in the basement of the 20th Medical Group facility. (b) The employer did not affix or post labels or signs to notify employees of the presence of asbestos-containing floor tile located in the basement hallway of the 20th Medical Group facility. NOTE: Since it is generally not feasible to put labels on floors, alternatives may be used. If asbestos-containing floors are being serviced by employees using a common equipment room day after day, then a sign or label for the asbestos flooring can be posted in the equipment room. The object is to forewarn employees who may be potentially exposed during the floor cleaning operation and have access to the material. ABATEMENT CERTIFICATION IS REQUIRED
