29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. Slip Sheet Department - On or about January 14, 2016, in the facility, employees required to flush out sediment from a heat exchanger through a bypass hose on Line No. 1 were exposed to steam and water. The employer did not ensure that employees were provided and used suitable protective clothing to protect against thermal burn injuries.

29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: Slip Sheet Department- On or about January 14, 2016, in the facility, employees were required to flush out sediment from a heat exchanger through a bypass hose on Line No. 1 were exposed to steam and water. 29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about January 14, 2016, the employer did not contact OSHA within 24 hours when an employee at there facility sustained a work-related injury.
