29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and/or updated annually:    (a)  UHS of Centennial Peaks LLC, dba Centennial Peaks Hospital, at 2255 S 88th Street, Louisville, CO: On and preceding 1/28/16, the employer did not ensure that the Exposure Control Plan was reviewed and/or updated annually.  Employees performed nursing and other healthcare duties at Centennial Peaks Hospital, including but not limited to injection of medications using syringes with needles, blood sugar checks using lancets, blood draws, and administration of medication using IV's.  The employer determined that employees were potentially exposed to blood or other potentially infectious materials (OPIM).  Portions of the Exposure Control Plan had been reviewed in 2015.  The employer's last documented complete review of the Exposure Control Plan was February 2014.  This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employees exposure:    (a)  UHS of Centennial Peaks LLC, dba Centennial Peaks Hospital, at 2255 S 88th Street, Louisville, CO: On and preceding 1/28/16, the employer did not ensure that engineering and work practice controls were used to eliminate or minimize employees exposure.  Employees performed nursing and other healthcare duties at Centennial Peaks Hospital, including but not limited to injection of medications using syringes with needles, blood sugar checks using lancets, blood draws, and administration of medication using IV's.  Nurses drew blood from central line ports through Huber needles and tubing using BD Luer-Lok tip syringes with the needle removed.  Following the blood draw, a needle was attached to the syringe, and while holding the vacutainer, the blood was transferred from the syringe into the vacutainer.  The employer did not utilize engineering controls to eliminate or minimize exposure during blood draws through central line ports.  This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): During use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found:  (a)  UHS of Centennial Peaks LLC, dba Centennial Peaks Hospital, at 2255 S 88th Street, Louisville, CO: On and preceding 1/28/16, the employer did not ensure that containers for contaminated sharps were easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found.  Employees performed nursing and other healthcare duties at Centennial Peaks Hospital, including but not limited to injection of medications using syringes with needles, blood sugar checks using lancets, blood draws, and administration of medication using IV's.  Nurses administered multiple sedatives using BD Luer-Lok syringes with needles to patients in Quiet Rooms.  Following injections, the nurses transported multiple contaminated sharps to sharps containers located at nurse's stations, approximately fifty six feet away, for disposal.  The employer did not ensure that sharps containers were easily accessible or located as close as feasible.  This condition exposed employees to a bloodborne pathogen hazard.  (b)  UHS of Centennial Peaks LLC, dba Centennial Peaks Hospital, at 2255 S 88th Street, Louisville, CO: On and preceding 1/28/16, the employer did not ensure that containers for contaminated sharps were easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found.  Employees performed nursing and other healthcare duties at Centennial Peaks Hospital, including but not limited to injection of medications using syringes with needles, blood sugar checks using lancets, blood draws, and administration of medication using IV's.  Nurses administered insulin using BD Luer-Lok syringes to patients outside Med Rooms.  Following injections, the nurses transported the contaminated sharps to sharps containers located within the Med Rooms, behind locked doors, for disposal.  The employer did not ensure that sharps containers were easily accessible or located as close as feasible.  This condition exposed employees to a bloodborne pathogen hazard.

On 3/20/15 a nurse incurred an injury (needlestick) while handling a contaminated syringe with needle.  The employer had not established or maintained a sharps log.
