29 CFR 1910.1001(j)(5)(i): The building owner or employer did not affix or post labels or signs to notify employees of what materials contain ACM and/or PACM for previously installed ACM and/or PACM:  (a)  The employer did not affix or post labels or signs to notify employees of the presence of asbestos-containing floor tile located in the basement of the 20th Medical Group facility.   NOTE: Since it is generally not feasible to put labels on floors, alternatives may be used. If asbestos-containing floors are being serviced by employees using a common equipment room day after day, then a sign or label for the asbestos flooring can be posted in the equipment room. The object is to forewarn employees who may be potentially exposed during the floor cleaning operation and have access to the material.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes:  (a)  The employer did not provide employees who performed floor stripping/buffing on asbestos-containing floor tile, an asbestos awareness training course as indicated above.   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) The employer did not maintain a copy of their written hazard communication program at the worksite where employees were potentially exposed to hazardous chemicals contained in floor strippers, disinfecting solutions, and cleaners.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  (a) A housekeeping employee who was potentially exposed to hazardous chemicals (e.g., disinfectant solutions, cleaners, etc.) had not been informed about the location and availability of the employer's written hazard communication program, or trained on the details of the program.  (b) As of 1/22/16, housekeeping employees had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs).    ABATEMENT CERTIFICATION IS REQUIRED
