29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) On or about January 25, 2016 employees were utilizing a flexible electrical cord to energize battery chargers for the cordless screw guns they were using, and portable 7-1/4" Skill circular saw, and were not protected from electrical shock hazards because the ground pin was broken/removed from the flexible cord.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a). On or about January 25, 2016 employees were working on a one story, low pitch, residential roof project to secure existing roof panels, and repair the ridge cap, and were exposed to falls in excess of 10 feet to the ground below, because they were not protected from falling by means of personal fall protection, safety nets or guard rail systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about January 25, 2016, employees had utilized an aluminum extension ladder to access a one story roof and had not placed or positioned the ladder with the side rails 3 feet above the upper landing surface.
