29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge. Employees engaged in framing operations were exposed to fall hazards in excess of 11 feet while using stairs without a hand rail on the open side to access the upper levels.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. Employees engaged in framing construction activities were exposed to fall hazards in excess of six feet while working from a boom lift without fall protection. The employer, Go Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(2)(i), which was contained in OSHA inspection number 1015995, citation number one, item one and was affirmed as a final order on February 6 2015, with respect to a workplace located at 12th St. Big Spring, TX.
