29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     (a) North side, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127. Employee was exposed to eye injuries while operating a pneumatic nail gun at 120 PSI with no protective eyewear. The employee installing exterior sheathing was not provided with safety goggles or glasses and was exposed to struck-by hazards which included plastic collated debris and fasteners.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Top plate, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127: Employee was exposed to falls of approximately 10-feet, 11-inches while engaged in framing activities on the top plate with no fall protection system in place.     (b) First floor, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127: Employees were exposed to falls of approximately 10-feet, 5-inches to the lower level while engaged in framing activities on the first floor walking/working surface which was not equipped with guardrails on the door and window openings.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    (a) North side, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127: Employee was exposed to fall hazards over 4-feet while working from a Werner (Model - NXT1A06) Step Ladder (A-Frame Style) in the closed position as this ladder was designed to be used in the opened position with the spreaders extended.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) First level, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127: The employer failed to inspect electrical cords which was used to power a circular saw. The cord had a torn casing in the location of the strain relief which exposed the internal insulated circuits and was not removed from service or repaired.  (b) Temporary service box on front side, new residential construction located at 2428 Stienbeck Lane, Powder Springs, GA 30127. The employer failed to inspect electrical equipment on site including the cord to the Jenny Air Compressor as it was missing a ground pin and the employer failed to replace it or remove it from service.
