Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards when elevated in a pallet/wood work platform with a power industrial truck:    (a) Stockman's Farm Supply, Osseo, WI warehouse; on or about December 30, 2015, workers were exposed to fall hazards while elevated in a wooden personnel work platform that was not constructed and rated for use, such as but not limited to:    (1) The work platform was approximately 37 inches above the power industrial truck forks,  (2) The work platform was not securely attached the carriage or forks,   (3) The maximal load including personnel and equipment was not prominently indicated on the work platform,  (4) The weight of the empty platform was not prominently indicated on the work platform, and  (5) The minimum capacity of the truck on which the platform can be used was not prominently indicated on the work platform.
