29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: On or about December 18, 2015 and in times prior thereto worker put a ladder in place without securing it. The ladder was located at the roof access area and created a hazard of the ladder becoming displaced and falling to the 4th floor.
