29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process: On January 16, 2016 and at times prior thereto, at the Peroxychem facility in Pasadena, TX, the employer did not develop and implement written procedures to remove work solution (WS) and hydrogen peroxide mixture from T-4031 and then transfer the removed work solution (WS) to the SM tanks.

On January 16, 2016 and at times prior thereto, at the Peroxychem facility in Pasadena, TX, the contract employer did not assure that each of its employees were trained in the work practices necessary to safely skim Work Solution (WS) and hydrogen peroxide mixture from T-4031 and transfer the Work Solution (WS) to the SM tanks.
