29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) In the Concentra Health Services, Inc. facility located at 5601 Greenville Avenue, Dallas, Texas where the employer did not provide employees who used glutaraldehyde with chemical splash proof goggles when pouring glutaraldehyde out of a bottle during disinfection operations.

29 CFR 1910.157(b)(2): The employer did not have an emergency action plan meeting the requirements of 1910.38 which designated certain employees to be the only employees authorized to use available portable fire extinguishers, and which required all other employees in the fire area to be immediately evacuated from the affected area upon the sounding of the fire alarm:       (a) In the Concentra Health Services, Inc. facility located at 5601 Greenville Avenue, Dallas, Texas where the employer did not review the emergency action plan with each employee who was required to evacuate and/or use portable fire extinguishers in the case of a fire.
