29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv): (a) On or about November 9, 2015, at the preparation site for the Philips 66 Methanol tanks, employee training did not include how to identify hazardous chemicals encountered in the workplace, the physical and health hazards of chemicals in the workplace, the specific procedures to protect the employee from hazardous chemical exposures, and emergency procedures. Employees were exposed to unknown organic vapors when vacuuming pipes.
