29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: (a) Blow Mold - On or about February 2, 2016 employee operating the blow mold was exposed to noise at 81.9% of the permissible daily noise (8-hour time weighted average sound level of 90 dBA) or an equivalent sound level of approximately 88.5 dBA during the 400 minute sampling period on 2-2-16. Exposure calculation included a zero increment for the time not sampled.
