29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms. Handrails were not provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails were not installed in accordance with the provisions of 1910.23:  a)  Inside the Front Entryway - On or about January 21, 2016, employees, accessing the second floor with the main staircase, were exposed to a fall hazard of 10 feet down the stairs from the second floor to the front entryway because the handrail was not continuous to the second floor.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)  In the Dental Lab - On or about January 21, 2016, the employer had not provided employees with effective information and training on the chemical-specific hazards associated with but not limited to Metallic Dental Alloys containing Palladium, Silver, Nickel, Chromium, Gold, and Cobalt; Quartz dental appliances; and Respirable Dust (PNOR - Particulates Not Otherwise Regulated) at the time of their assignment to the dental lab, creating and finishing crowns and other dental implants.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a)  Throughout the facility - On or about January 21, 2016, the employer had not conducted training on the revised HCS 2012 shipping container labels, the hazard pictograms, and safety data sheet organization for the workplace hazardous chemicals, such as but not limited to Metallic Dental Alloys containing Palladium, Silver, Nickel, Chromium, Gold, and Cobalt; Quartz dental appliances; and Respirable Dust (PNOR - Particulates Not Otherwise Regulated) so that the employees could find and apply the [M]SDS hazard information.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
