29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   On or about January 27, 2016, at the north side of the commercial construction where employees were exposed to a fall hazard when using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about January 27, 2016, on the north and east side of the commercial construction where two employees were exposed to a fall hazard greater than 6 feet when setting copings on the top of the roof without a fall protection system.    Francisco Rosales. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.501(b)(1), which was contained in OSHA inspection number 1056292 Citation Number 1, Item Number 1 and was affirmed as a final order on 06/19/2015, with a final abatement date of April 17, 2015, with respect to a workplace located at 440 Cobia Drive, Katy TX 77494.
