29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: In the facility employees operating Hyster forklifts (#LT-109 and #OTR-2509) and those in close proximity were exposed to struck-by hazards. The employees were exposed in that the control lever(s) were missing markings to avoid moving the levers in an unintended direction and striking someone or something.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: On or about 01/21/16, in the basement an employee troubling shooting a noise on the cooling conveyor belt was exposed to amputations. The employee was exposed in that the guard on the chain and sprocket for the conveyor was not secured to the piece of equipment.
