29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: (a) Date of Violation: On or about 01-27-2016 and times prior thereto, located at BAE Systems Norfolk, Building #202, Norfolk, VA 23523, Condition: A 50 foot temporary extension cord was plugged directly into another 50 foot temporary extension cord on an unprotected GFCI circuit and was used to provide temporary power to various powered electrical hand tools on top of the scaffolding unit while conducting interior roof construction activities. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. (a) Date of Violation: On or about 01-27-2016 and times prior thereto, located at BAE Systems Norfolk, Building #202, Norfolk, VA 23523, Condition: The employer did not ensure that employees using portable powered electrical equipment on top of the scaffold system were not protected by a GFCI circuit while conducting interior roof construction activities. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: (a) Date of Violation: On or about 01-27-2016 and times prior thereto, located at BAE Systems Norfolk, Building #202, Norfolk, VA 23523, Condition: Employees were exposed to electrical shock because of electrical conductors in the motor were unguarded and uncovered on the Hougan Brand, Model#HMD904, 115 VAC 50/60HZ 8.0 AMP Magnet Drill. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
