29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) Temporary Power at Jobsite- on or about January 26, 2016, Dees Electrical, Inc. installed an 4-inch X 4-inch electrical handy box that was designed to be secured and mounted.  The electrical handy box was lying on the ground and hard wired to the temporary power pole.  The electrical handy box was used by the siding sub-contractor employees to support electric power to hand tools, thereby exposing sub-contractor employees to shock and electrical hazards.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.  a) Temporary Power Panel - On or about January 26, 2016, employees were allowed to use 4-inch X 4-inch electrical  handy box that was designed to be secured and mounted without the use of a Ground Fault Circuit Interrupters (GFCI), exposing employees to shock and electrocution hazards.
