29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Shop - RAS Turbobend folding machine, type 61.31and number 62/6, had an unguarded point of operation, on or about January 22, 2016. An employee crushed three fingers in the machine when he initiated a cycle with the foot pedal.
