29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Jobsite Unit 3 - On or about January 26, 2016, a 4" x 2 1/8" x 1 7/8" metal handy box designed to be secured and mounted was left loose and fitted with non-metallic wire, exposing employees to shock, electrical and life safety hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) Jobsite - On or about January 26, 2016, employees were using a portable electric power tools (Milwaukie Super Hawg) that was plugged into a black power cord that did not allow for strain-relief.
