29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):  (a)  On or about January 20, 2016, employees working from a scaffold without the benefit of a fall protection system, were exposed to falls of approximately 14 feet.

29 CFR 1926.706(b): Masonry walls over eight feet in height were not adequately braced to prevent overturning and to prevent collapse until permanent supporting elements of the structure were in place:   (a)  On or about January 20, 2016, employees were exposed to crushing and struck-by hazards where the mason constructed wall sections, which each measured approximately 30 feet high and 50 feet in length had only one temporary support installed.    Abatement note:     Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of Standard Practice for Bracing Masonry Walls Under Construction, dated December 2012, which states:     A 1.3-  Utilize two braces per panel with each brace in 0.2 times the panel length from the edge or 5 feet.
